Job Description Job Description Position Summary: The Intensive Care Coordinator (ICC) is responsible for coordinating individualized, community-based services for children and adolescents with Serious Emotional Disturbances (SED. Using a wraparound, family-centered approach, the ICC facilitates team-based planning, service coordination, and cross-system collaboration to support youth and families in achieving stability, improved functioning, and meaningful life goals. The ICC ensures that care is well-integrated across providers and tailored to each youth's strengths, needs, and preferences. This position may involve working with youth at high or low intensity levels, depending on the complexity of their needs. Key Responsibilities: Lead the development and implementation of individualized care plans using a high-fidelity wraparound process. Facilitate regular Child and Family Team (CFT) meetings with families, service providers, and natural supports to coordinate care and revise service plans as needed. (For High Intensity Youth only). Conduct comprehensive assessments and strengths-based planning with youth and families. Act as the primary point of contact and advocate for youth and families in navigating mental health, educational, legal, and social service systems. Coordinate and monitor services and supports across multiple systems and providers. Ensure timely documentation of assessments, service plans, progress notes, and communication in compliance with agency and regulatory standards. Identify and address gaps in services, advocate for needed resources, and promote culturally responsive and trauma-informed care. Maintain regular contact with families and providers to monitor progress, promote engagement, and support family voice and choice throughout the care planning process. Participate in supervision, team meetings, and ongoing training. All other duties as assigned. Qualifications: Bachelor’s degree in social work, psychology, human services, education, or related field (required). Minimum of one year of experience working with children or adolescents with SED or other high needs and their families (preferred). Familiarity with and ability to navigate and document within Electronic Health Records Systems. Familiarity with wraparound planning, systems of care, and trauma-informed practices is strongly preferred. Strong organizational, communication, and interpersonal skills. Ability to work collaboratively and respectfully with individuals from diverse backgrounds and systems. Flexibility to travel and work in home, school, and community settings. Valid driver’s license, reliable transportation, and ability to travel regularly throughout the service area. Work Environment & Schedule: This is a non-exempt, full-time position. Standard hours are Monday through Friday, 8 AM – 5 PM CST This position requires in-person work and is not eligible for remote or hybrid work arrangements. This position involves community-based work and requires the ability to meet clients in various settings, including homes and schools, which may not be fully accessible. The role includes periods of sitting, driving, and walking, and may occasionally require lifting up to 25 pounds. Transportation Requirements: Provide transportation for consumers as needed using agency or personal vehicles. Maintain a safe driving record and valid driver’s license with personal liability insurance. Follow all agency transportation policies, including seatbelt usage, safe cellphone practices, and vehicle documentation. Immediately report any vehicle issues, accidents, or violations to appropriate personnel. In-Service Training & Continued Education Requirements: Participate in all required agency in-service and ADMH trainings Commitment to ongoing professional development to enhance skills and stay informed on best practices in behavioral health. Completion of onboarding and any continuing education requirements as directed by the agency or supervisor. Ongoing professional development to maintain best practices and compliance with evolving standards of care.
