Overview: Join Our Team as a Registered Nurse - IntegrationIs it

are committed to creating meaningful patient experiences. -In this
role, you'll be responsible for supporting the integration team
with compassionate patient care at de novo locations across the
region. You'll work collaboratively with the care team and assist
with training the staff of the acquired location. This role
requires extensive travel and on-call responsibilities. Most
importantly, you will help ensure all patients are receiving
consistent quality care and that care team members have the support
they need to effectively manage the transition. -And just like all

A heart to serve patients and
families and a passion for providing the best possible
careEducation: Graduate of an accredited school of nursing with a
current state license as a registered nurse (compact/multi-state
license required)Experience: 2+ years of nursing experience in a
clinical care setting (hospice experience required)Required:
Reliable transportation. Ability to sit, stand, bend, move
intermittently and lift at least 25 lbs and bear the weight of an
